The greatest amount of evidence for the therapeutic effects of cannabis relates to its ability to reduce and alleviate a wide range of ailments including chronic pain, nausea and vomiting due to chemotherapy, and spasticity (tight or stiff muscles) from Multiple sclerosis. Our doctors are educated and experienced, they are available to answer any questions you may have regarding MMJ, its benefits and questions regarding the certification process.

    1. Changes to the state process now require the following, prior to your appointment please set up your Patient Portal Login. This information will be needed prior to processing your application.     2. Complete your intake paperwork by clicking this link: Intake paperwork link here
    3. Complete your patient attestation form by clicking this Attest
    4. If you have a current MMJ Card, please upload below

    5. If you have valid medical records, please upload below (Records must be current within the past 12 months, and state a qualifying condition.)
    6. Upload a copy of your Arizona ID/DL or US passport below
    7. Upload a photo of yourself below (Photo must be taken from Shoulder up, top of your head must be visible. No glasses on the face or head.)
    8. If you have a valid Food stamps proof, please upload below. (Must state your name printed on the FRONT of the card, if it is not printed on the front we will need your most recent approval letter dated within 6 months of the date your being seen.)
    9. Lastly, we will need payment information. (You can email this to Support@ezvisitmd.com or we can take information over the phone.)




    Once all the steps are completed and it is time for your appointment, you are ready to enter the patient waiting room!
